A heat pump blowing cold air while the thermostat says heat is a fault at the outdoor unit — ice on the coil, a reversing valve stuck in cooling, or a system that has leaked refrigerant. Emergency heat is the hold until that fault is repaired, not a reason to skip the call. The U.S. Department of Energy describes a heat pump as a machine that moves heat. In heat mode that heat has to come from outdoor air. When the coil is a block of ice, or the four-way valve never slides, the air at your register in Sacramento stays cold.

The three causes that actually blow cold air
Ice that never clears. A light frost that melts during defrost is normal. A solid ice shell after a cycle, or ice that returns within an hour, is a failed defrost sensor, a stuck reversing valve, low refrigerant, or blocked outdoor airflow — leaves, a still fan, a coil packed with debris. In heat mode the outdoor coil is the evaporator. A packed indoor filter does not ice it. Sacramento Tule fog and 30°F nights will ice an outdoor coil faster than a dry mountain night.
A reversing valve stuck in cooling. The house asked for heat. The outdoor unit is still sending refrigerant the summer direction. Supply air feels like a weak AC. That is the same mechanical fault as a valve stuck on a 101°F day, only you notice it when you want warmth. Do not keep flipping modes.
Low refrigerant. A slow leak drops suction pressure, the outdoor coil ices, and the air never warms. Topping off without finding the leak is a patch. A licensed recovery, leak check, and recharge is the repair.
Replace a packed indoor filter anyway — it cuts heat at the registers — but do not treat it as the reason the outdoor coil is a block of ice.
What to do in the next ten minutes
Set the thermostat to heat, fan auto, and wait. A heat pump’s 90–110°F supply air can feel cool next to a furnace. Judge the house, not the hand: if rooms reach the set point, that air is doing its job.
Look at the outdoor unit from the yard. If it is a white block of ice, leave it. Do not pour water on a running fan and do not chip the fins. Clear leaves and toys from the sides so the next defrost has air to work with.
The outdoor coil exhausting cold air in heat mode is normal — it is taking heat from the yard. Switch to emergency heat and book heat pump repair if rooms never reach the set point, or if that coil is a slab of ice. Strips are the fallback until a tech can get there. They are not a diagnosis.
Why Sacramento winters do this
Valley fog plus overnight lows in the 30s put a heat pump in defrost more often than a coastal unit. SMUD on-peak windows make emergency heat expensive. That is still the right hold when the outdoor unit is a slab of ice.
We opened a 2014 heat pump behind a 1970s ranch in Sacramento last January. The owner had run emergency heat for two nights — correct, the outdoor coil was a solid slab. After a thaw and a defrost-sensor swap, the unit heated. The strips went back to off.
